Access Health Professionals is an accomplished team of doctors and advanced practice providers who utilize years of experience in caring for patients in the post-acute and long-term care settings. Our providers are knowledgeable and compassionate in treating a patient’s unique needs. This patient-centered approach results in the highest level of care achievable outside of a hospital setting. Whether our providers are managing your medical needs in a skilled nursing facility, long-term or memory care unit, or in the assisted living community, you can be assured that you have outstanding access to quality medical care.
Skilled Nursing / Rehabilitation
Our group of multi-specialty physicians, physician assistants, and nurse practitioners realize the tremendous need for continuous and quality oversight during this difficult and often confusing transition and are experienced and proactive in providing near hospital-level care. Our providers deliver consistent and regular on-site presence and immediate access for nursing staff. When highly acute patient needs are adequately managed by our medical team in collaboration with the facility, patients avoid re-admission to the hospital and can focus on their recovery.
Long-Term Care & Memory Care
Assisted Living Communities
Access Health Professionals strives to help keep assisted living residents healthy and independent. Our medical providers begin with an in-depth consultation that includes a complete medical review and assessment of the whole person. We are then able to develop comprehensive solutions to meet the individual needs of each patient we see.
Access Health Professionals providers typically schedule weekly on-site clinical hours at assisted living locations. As well, an Access Health Professionals provider is available on-call 24 hours per day, 7 days per week, to address urgent medical issues.
