Access Health Professionals includes a group of multi-specialty physicians and advance practice providers with significant experience in high-acuity inpatient care, outpatient and post-acute care. We collaborate with multiple sub-specialists to produce the highest level of post-acute care. We are skilled to provide the continuum of care each patient requires on his or her path to recovery, from the hospital setting to the post-acute transition.
Dr. Anzaldua attended St. Mary’s University in San Antonio, Texas, for his undergraduate studies. He obtained his medical degree from Saba University School of Medicine, then completed his Internal Medicine residency at St. Vincent Hospital in Indianapolis, Indiana. Following residency, he completed his General and Interventional Nephrology Fellowship at St. Louis University in St. Louis, Missouri. He is board certified in Internal Medicine and Nephrology by the ABIM. He is the owner of Access Health Professionals.
Dr. Armstrong attended Earlham College in Richmond, Indiana, for his undergraduate studies. He obtained his medical degree from Indiana University School of Medicine, and followed that with a family practice internship with Indiana University Medical Center. He did his Internal Medicine residency at St. Vincent Hospital. He has held board certifications in Internal Medicine and Geriatric Medicine.
Dr. Armstrong has specialized in Internal Medicine since 1987. He currently serves as a hospitalist at St. Vincent Hospital in Indianapolis. He has managed patients in the nursing home setting his entire professional career.
Senior Adult Nurse Practitioner
Ms. Arnett obtained Indiana University and obtained her Bachelor of Science in Nursing. She graduated with her Masters of Science in Nursing and an Adult Nurse Practitioner degree from Indiana University-Purdue University, Indianapolis. Ms. Arnett specializes in diabetes and endocrine disorders, kidney disease, chronic illness, infectious disease and acute and rehabilitative care. She is a member of the American Academy of Nurse Practitioners and the American Medical Directors Association (AMDA).
Ms. Armstrong completed her Bachelor of Science degree from Marian University and then completed her Physician Assistant studies at Butler University in Indianapolis. She previously collaborated with the Hospitalist team at IU Health Methodist, where she provided care across the continuum from admission, to patient evaluation and treatment.
Adult-Gerontology Nurse Practitioner
Ms. Titus-Quear attended Indiana University-Purdue University Indianapolis where she obtained her Bachelor of Science in Nursing. She graduated with her Masters of Science in Nursing and an Adult-Geriatric Nurse Practitioner degree from Purdue University. She has more than ten years RN clinical experience in the acute care setting, including medical-surgical, progressive care and intensive care units. She is a member of the Gerontological Advanced Practice Nurses Association (GAPNA) and the American Association of Nurse Practitioners (AANP).
Ms. Schultz completed her Bachelor of Science in Biology degree at Indiana University in Bloomington. She then received her Master of Physician Assistant Studies degree with an Internal Medicine concentration at Bethel University in Tennessee.
Adult-Gerontology Nurse Practitioner
Ms. Pitts completed her Bachelor of Science degree from Indiana University Kokomo. She went on to finish her Masters of Science in Nursing and an Adult-Geriatric Nurse Practitioner degree from Purdue University. She was previously a nurse who worked in multiple clinical settings over the last ten years, including long term care, medical surgical, and progressive care unit.
Adult Nurse Practitioner
Ms. Bell is an Adult Nurse Practitioner who received her Bachelor of Science in Nursing from Marian College and her Masters of Science in Nursing from Ball State University. She has vast RN clinical experience in the acute care settings, including interventional radiology, nuclear medicine (cardiac testing), trauma, emergency unit care, cardiac progressive and surgical care, and endocrinology and dialysis patient care.
Ms. Joest is a licensed practice nurse who has more than thirty years of professional nursing experience, both in the office setting and in nursing facilities. She has focused the last twenty years of her practice in geriatric care. Ms. Joest provides clinical and administrative support to the providers in our group. Ms. Joest is also certified as a professional coder with the AAPC. She trains and audits Access Health Professional providers to ensure proper clinical documentation and coding of our patients’ charts.
Chief Officer of Operations and Legal Affairs
Ms. Lutes serves as the leader of operations and legal services for Access Health Professionals. She obtained her Bachelor of Science in Biology from Indiana University and her juris doctorate, cum laude, from Indiana University McKinney School of Law. Prior to joining AHP, Ms. Lutes worked for almost 15 years in private practice law, concentrating on insurance defense coverage and litigation matters. She handled state and federal trials, hearings, motions practice, mediations, and arbitrations. She is a member of the Indianapolis Bar Association and Indiana Medical Group Management Association.
Patient Care Coordinator
Ms. Kress has been with the AHP since inception, where she serves as the Patient Care Coordinator. Originally from Indianapolis, she graduated from Indiana University with a degree in English. She also has a degree in Theatre Arts from Indiana University. Ms. Kress worked for several years in the not-for-profit arts sector.
Ms. Schwegman attended Indiana University where she obtained a degree in psychology. Ms. Schwegman has worked with our group as a Medical Assistant since 2017, providing administrative support to our physicians and advance practice providers. She also regularly communicates with patients, family members, and the staff members of the facilities where we treat patients.
