Skilled Nursing / Rehabilitation
Our group of multi-specialty physicians, physician assistants, and nurse practitioners realize the tremendous need for continuous and quality oversight during this difficult and often confusing transition and are experienced and proactive in providing near hospital-level care. Our providers deliver consistent and regular on-site presence and immediate access for nursing staff. When highly acute patient needs are adequately managed by our medical team in collaboration with the facility, patients avoid re-admission to the hospital and can focus on their recovery.

Our medical providers begin with an in-depth consultation that includes a complete medical review and assessment of the whole person. We are then able to develop comprehensive solutions to meet the individual needs of each patient we see.
Access Health Professionals providers typically schedule weekly on-site clinical hours at assisted living locations. As well, an Access Health Professionals provider is available on-call 24 hours per day, 7 days per week, to address urgent medical issues.
